Your AI assistant, or hers
Ask your own assistant about Mom’s medicines
You can switch it on for her profile. If you help manage Mom’s profile, you can connect your own Claude or ChatGPT to it and choose what it can see. If she already uses Claude, ChatGPT or Grok, she can still connect her own.

Within what the connection allows, the assistant can read her medicines, records, labs and the summary of her history. With write access it can update her medication list and set reminders. It works only with what’s in Triangle, and can’t change anything at a hospital or clinic.
Questions you might ask it
- What is each of Mom’s medicines for, in plain words?
- When was her amlodipine last filled?
- Add the vitamin D she takes every morning to her list.
The connector address
https://app.trianglehealth.com/mcp- Copy the address.
- In your assistant’s settings, add a new connector with it.
- Sign in to Triangle when asked.
- Choose what it can read or change, pick Mom’s profile to share, then tap Accept.
In Claude
Settings→Connectors→Add custom connector→paste the address
A Triangle sign-in appears the first time Claude uses a Triangle tool.
In ChatGPT
Plugins→+→Add custom MCP server
Enter a name and the address, choose OAuth, then Create as a plugin and sign in to Triangle. Needs Business, Enterprise or Edu, or Pro for read-only.
In Grok
Skills and Connectors→New Connector→Custom
Enter a name and the address, sign in to Triangle and accept. Needs a paid Grok plan.
Once connected, what an assistant reads is handled under that assistant’s own privacy policy, which Triangle doesn’t control, so connect only an assistant you trust. You and Mom each see and disconnect your own connections in Triangle under Settings → AI service connections, and access stops immediately.
